Mobility WA provides NDIS Support Coordination, Specialist Support Coordination, Psychosocial Recovery Coaching, Psychosocial Functional Capacity Assessments and Social Work Services.

We aim to empower individuals with NDIS funding to maximize their plans' potential through, creative and flexible approaches.

Our primary goal is to help participants and their supports to navigate the NDIS landscape effectively.

We facilitate tailored solutions that align with individual goals and aspirations, by leveraging diverse support options and thinking innovatively.

Our Services

Support Coordination

We will:

  • Assist participants in understanding their NDIS plans

  • Connect participants with suitable service providers

  • Monitor plan budgets and support effectiveness

  • Advocate for participants' rights and preferences

  • Empower participants to maximise their NDIS Plan

  • Engage positively with formal and informal supports

  • Help make changes to your plan if goals change or needs are not being met

  • Walk side by side with you and your supports

Specialist Support Coordination

We will:

  • Provide higher-level support for complex needs

  • Social Worker level case support

  • Manage challenges in support environments

  • Manage and support crisis situations

  • Provide support documentation for services ie Justice, Housing, Mental Health etc

  • Organise and attend stakeholder meetings to support NDIS Plans

  • Ensure consistent service delivery and plan outcomes

Psychosocial Functional Capacity Assessments

We will:

  • Develop an NDIS Psychosocial Functional Capacity Assessment

  • Provide a Social Worker to complete an FCA with a participant who has a psychosocial disability or FASD, to better inform their NDIS plan needs

  • Incorporate all aspects of a participants life and daily functioning to understand an individual’s needs holistically

  • Utilise evidence based Assessment Tools to capture daily needs

  • A minimum of 2 meetings with a participant is required to gather information and assess need

Psychosocial Recovery Coaching

  • Building Recovery-Enabling Relationships to work with you NDIS goals

  • Recovery Planning buy developing a comprehensive plan to build strategies and resilience

  • Coaching and Skill Development in life skills, motivation, recovery focused resilience and decision making

  • Support Coordination

  • Collaboration with supports, relevant professionals, family members, to create holistic approach to support

  • Documentation and Reporting

  • Advocacy

Capacity Building

We will:

  • Develop participants' skills to navigate the NDIS system

What is a Functional Capacity Assessment?

  • An NDIS Psychosocial Functional Capacity Assessment will outline to the NDIS what supports an individual needs to participate in daily life and the community.

  • These are completed by an OT or Social Worker to enhance your NDIS plan and highlight all the supports you need on a day to day basis in order to live your best life.

  • A Social Worker will use a holistic person-centred approach for participants with a psychosocial disability or FASD, to better inform their NDIS plan using recognised assessment tools to capture your needs.

  • Incorporating all aspects of a participants life and daily functioning, an effective Psychosocial FCA will be person-centred, identify marginalisation, social factors and systemic barriers when understanding an individual’s needs.

Biography

Biography

My name is Felicity Lindsay. I am a dedicated Social Worker with over 20 years experience working in the Community Services Sector including: Mental Health, Disability, Foster Care, Therapeutic Residential Care, Case Management and Training and Practice.

I have worked throughout Western Australia and have a strong understanding of the needs of the community in WA, and in particular the needs of people with a disability, those suffering adversity, mental health and diverse community groups. This is what lead me to start up Mobility WA. That, and having my own son with a complex disability, so I know first hand how individuals and their families are challenged everyday to access the services and supports they need.

My goal is to ensure people with NDIS funding have, firstly, the funding they need to live their best life, and secondly, to ensure they are linked in with high quality services that put the person first.

I am extremely passionate about building strong trusting relationships with people I work for - identifying strengths and building on the collective power of community. When I'm not working you will find me in my garden, on a weekend getaway or spending time with family.

I acknowledge the Traditional Owners and Custodians of the lands on which we live and pay our respects to Indigenous Elders past, present and emerging. Sovereignty has never been ceded. It always was and always will be, Aboriginal land.

No aspect of identity exists in isolation - we consider the whole person.

A Support Coordinator will help you:

  • Understand your plan

    Support coordinators help people understand their NDIS plan, including their budget and how to manage it 

  • Connect with services

    Support coordinators help people connect with NDIS providers, community services, and other government services 

  • Build skills

    Support coordinators help people build confidence and skills to use and coordinate their supports 

  • Address challenges

    Support coordinators help people address unexpected events or challenges, such as moving house or going to hospital 

Support Coordination can help people: Live more independently, increase their skills, be included in their community and in employment, and take greater control of achieving their goals. 

The NDIA may fund support coordination in an NDIS plan, depending on the individual's needs and circumstances. You may need to request this. There is also a higher level of support coordination called specialist support coordination, which is for people with more complex situations.

We are located in Fremantle Western Australia, but can provide support for people living all over WA. In the metro area face to face, and via video link, or telelink if you live remotely. If you live remotely we will endeavour to meet you at least once face to face during the course of your plan.

Your funding will either be self managed, plan managed or NDIA-managed.

Self-managed; receive funds into your nominated bank account (or receive reimbursement) from your NDIS plan by making an online payment request on the myplace portal which you can then use to make payment.

Plan-managed; we will invoice your plan manager who will make payment.

NDIA-managed; we will electronically claim online through PRODA (the myplace provider portal).

If your plan does not include specific funding for support coordination, you can still access these services by reallocating funds from other parts of your budget:

Core Budget: Funds allocated here can sometimes be used flexibly to support your needs.

Capacity Building Budget: You can use these funds to create a “support coordination package.”

Creating a Support Coordination Package

A support coordination package works like a service agreement with a provider, where you allocate a certain number of hours for support coordination services. This package is typically short-term, depending on the level of support required.
